Determinants of System Satisfaction with the National Health Insurance in Korea: Evidence from the Korea Welfare Panel Survey

Abstract
This study examines determining factors of system satisfaction with the National Health Insurance (NHI) in Korea using data from the Korea Welfare Panel Survey 2006. In both bivariate and multilvariate analyses, we uniformly find that income level, NHI enrollment, education level and self-rated health status are negatively related with the level of satisfaction whereas the emotional distress in daily living and having chronic diseases lead to lower satisfaction level. Further, the delay in premium payment statistically significantly depend on the satisfaction level in regard to whether the NHI plan appropriately provides financial protection for needed care. Also unstable employment type and voluntary payement seem to be related with not paying premium. There is, however, no evidence for attitude consistency among the expectation on the governmental contribution in health care financing, motivation in tax payment, and actual premium payment. Findings imply that income is the most critical factor in the system satisfaction and premium payment and that the satisfaction measures may not necessarily reflect the objective quality of the NHI plan which would be assessed through direct experience of service utilization. To fully address adequacy and performance effectiveness of the NHI system, further study should consider other relevant factors such as out-of-pocket payment and private insurance enrollment status.
